Levels of Hospice Care
Routine Home Care
Hospice care is provided in the comfort of the patient’s home or wherever the patient resides (SNF, ALF, and B&C) under the supervision of an Interdisciplinary Team of healthcare professionals. Routine Level of Care includes scheduled visits from skilled nurses, certified home health aides (CHHA), spiritual counselors (SC), medical social workers, and volunteers.
- Nurse: Will assess the patient and create a plan of care to provide individualized care based on each person’s unique needs. Obtain physician orders for treatment and medications. Educate families about disease processes, nutrition, medication indications, side effects, and administration. Monitor and perform wound care. On-call nurses are available for emergency assistance 24/7.
- CHHA: Will assist patients with daily living activities (hygiene, bathing, and more).
- Spiritual Counselor: Provide non-denominational spiritual care.
- Medical Social Workers: Assist families and patients with psychosocial needs (bereavement, legal paperwork, and more).
- Volunteers: Provides companionship (reading, playing games, watching TV, listening to music, and more).
General Inpatient Care (GIP)
Hospice level of care is defined as short-term care provided for a patient's pain management or acute or chronic symptom control that cannot be managed in other settings.
Continuous Care
- Provided during a period of crisis for management of acute medical symptoms in order to maintain the patient at home. Provided on a short-term basis when the patient needs more intensive care that is predominantly nursing care, for at least eight hours within a 24-hour period.
Respite Care
- Provides short-term relief for primary caregivers. It means having someone else look after your loved one while you take a break.
- Patients are placed in a hospice-contracted skilled nursing facility for up to 5 days.
